Description

Product Description:
The Chronic Obstructive Pulmonary Disease (COPD) Service allows high risk patients to securely share health data from connected hardware with healthcare professionals. The health data can be used to understand the likelihood of an exacerbation, allowing community intervention, avoiding unnecessary hospital admissions, improving quality of care and making better use of resources.
About Storm ID:
Storm ID is a digital transformation delivery partner for the healthcare sector. They developed Lenus, a system of interoperability, to bridge the gap between existing healthcare systems and new digital therapeutics and diagnostics through use of AI and machine learning. Lenus uses patient-generated health data to enable services that reduce rehospitalisation rates and waiting times and improve health outcomes. Services have been provisioned in the NHS to transform chronic condition care pathways for specialities like COPD, AF, dermatology and more. Prevalence of chronic conditions is increasing. The healthcare sector requires innovative solutions that use AI as part of routine care. For payers, Lenus reduces healthcare costs; for providers, Lenus increases customer engagement; and, for customers, Lenus delivers the type of digitally enabled health services that they are looking for.
Product Description:
Lenus enables patient generated health data in the form of physiology and patient reported outcomes from apps, wearables and sensors to be shared with health professionals and machine learning models as part of new care pathways. The platform offers a suite of FHIR Data APIs, identity, access and consent management services. A suite of EHR connectors provides healthcare organisations with a single point of integration for data gathered from patient facing digital health services and is compliant with OpenEHR.
