Health Quality Partners (HQP) are experts in the complexities and limitations of current health care delivery systems and has experience designing, implementing, and evaluating innovative care delivery systems that improve health outcomes for vulnerable populations – working collaboratively and in support of primary and specialty care services. We have worked with key stakeholders at all levels to support the implementation and adaptation of new care systems that are resilient to complex operational and financial challenges. Our design approach is grounded in evidence, including the lived experience of patients, caregivers, providers, and population health with randomized controlled trial data analyzed by knowledgeable staff or independent third parties. The HQP team is equally versed in the design, implementation, execution, and commercialization issues, bringing you firsthand expertise from senior levels of industry, clinical services, public health, and non-profit R&D sectors.

Health Quality Partners (HQP) is a 501(c)(3) non-profit R&D organization committed to designing, testing, and disseminating more effective systems of care for vulnerable patients. HQP's unique design and system thinking approach has proven to reduce suffering, improve health outcomes, and can lower the overall cost of health care. This has already been convincingly demonstrated among higher risk chronically ill older adults. As tested within the Medicare Coordinated Care Demonstration and with Aetna, HQP’s model of Community-based Advanced Preventive Care, significantly reduced hospitalizations, deaths, and total Part A & B Medicare expenditures.
