Guideway Care provides Care Guidance—care coordination services that fill in the gaps between healthcare visits—using tech-enabled, non-clinical staff, with a proprietary platform that extends nurse capacity. Care Guidance solves issues for patients, increases adherence to instructions, increases satisfaction and improves outcomes. Care Guidance identifies and resolves the personal issues hidden within social determinants of health that influence behavior. COVID-19 use cases:
Guideway’s tech-enabled Care Guidance services are designed to identify and resolve personal issues that affect the health and well-being of patients. As such, we were able to seamlessly pivot our services towards COVID-19 mitigation and follow-up efforts on behalf of our clients beginning in the earliest stages of the pandemic. Our COVID-19 services include: 1. Biweekly outreach for patients with conditions which place them at higher risk for complications due to COVID-19. Outreach includes patient education on self-isolation as well as symptom assessment to keep patients healthy and out of the ER. 2. Follow-up and monitoring for those who either tested positive or were under investigation for COVID-19, or who were discharged after an inpatient stay for COVID-19. Outreach includes symptom assessment/monitoring as well as education on self-isolating within the home.
