A unified digital front door, Empower provides patients a seamless clinical and financial experience to manage their health. It enables providers to create and execute digital clinical engagement campaigns, which will help drive organizational financial performance and quality outcomes. The solution is centered on patient and family outreach across care settings, from managing ambulatory-sensitive conditions to between-the-visits care and post-acute care. Our patient engagement tools have been implemented successfully at dozens of health systems of all sizes and varying levels of technology complexity. Empower extracts data from any number of source systems –such as EMRs, scheduling, labs, and payment information systems – integrating and linking data to deliver comprehensive health information to patients. In addition, the solution includes alerts and notifications and offers the ability to securely communicate with clinicians and care management staff. Empower meets the Office of the National Coordinator for Health Information Technology (ONC) 2015 Edition Health IT Certification Criteria.
