Chart Access provides secure access to patient records as well as provider messaging for organizations providing care outside of a health system. This solution is currently available for health systems utilizing Cerner Millennium clinicals. When a patient is discharged to a nursing home, skilled nursing unit, etc. those providers at the post discharge facility need an easy way to access hospital and/or clinic records for the patients at their post discharge facility. They also need a way to efficiently communicate with health system providers that are visiting patients at their facility. Today that requires a lot of paper and phone tag. Chart Access eliminates the need for paper exchange and playing phone tag.

At the point of admission, nH Identify classifies patients who are at a high risk for readmission or who have a high need for inpatient post-acute care. Using the data collected early in the patient stay, nH Identify provides a level of care recommendation reflecting the risks and needs of those patients. The tool allows hospital case managers to prioritize resources accordingly, manage acute length-of-stay, and reduce readmissions. This is crucial to improving patient outcomes by beginning each patient’s discharge process at admission and not letting high-risk patients slip through the cracks.
