About RoundingWell:
RoundingWell is a web-based patient engagement and clinical decision support solution delivered to patients as they transfer from one setting (such as a hospital) to another or as they are engaged by their clinician team in the management of chronic conditions. RoundingWell’s proactive outreach and communication with a patient evolves over time to focus on a variety of areas: discharge instruction understanding, medication and appointment adherence, sodium management, self care guideline understanding, caregiver support, and symptom awareness. Educational content, discharge reminders, and tailored messaging from clinicians are also delivered to patients via our “Health Check-Ins,” which can also be directed to a caregiver with the patient’s permission. RoundingWell applies patient responses and feedback against proprietary algorithms to identify the early warning signs of risk (medication non-adherence, fluid retention, ADL limitations, confusion, etc.) by aggregating, weighting, and evaluating all patient data gathered to date via our Check-Ins. RoundingWell then delivers this information into clinician hands at a time and in a manner that positions them to take informed, clinically appropriate, and cost-effective steps to manage a patient’s health; to avoid adverse events; to coordinate care; and to remain effectively engaged with patients and their caregivers.
