CancerNAV is a clinical application used to support patient navigators as they manage the treatment path for their oncology patient population through the health system. CancerNAV was originally developed at the Cleveland Clinic and is proven to increase navigator efficiency and improve patient access and overall quality of care.
About Family Care Path:
Family Care Path was formed in 2016 as a spin-off company from Cleveland Clinic Innovations with a mission to bring patient-benefiting products from the academic institute to market. They founded the company to introduce the web-based clinical decision support applications. CancerNav based on oncology navigation practice guidelines developed by the Tausig Cancer Center at Cleveland Clinic CancerNav allows a clinical or lay navigator to assist a patient through the complex care continuum of cancer treatment. The web-based SaaS program is highly customizable and can be integrated with Epic and is an app on Cerner. CancerNav is available as an app on the Salesforce.com HealthCloud for ease of use in the EMR environment. .
Product Description:

Guideway Care provides Care Guidance—care coordination services that fill in the gaps between healthcare visits—using tech-enabled, non-clinical staff, with a proprietary platform that extends nurse capacity.

Care Guidance solves issues for patients, increases adherence to instructions, increases satisfaction and improves outcomes.

Care Guidance identifies and resolves the personal issues hidden within social determinants of health that influence behavior. The ability of Care Guidance to address these non-clinical issues makes it a versatile tool for addressing two key types of patients—profitable patients with whom hospitals must build loyalty and adhesiveness, and negative margin patients with whom they must control costs by changing behavior. Guideway Care offers an effective, scalable solution that can help hospitals effectively engage these patients in order to increase patient loyalty and reduce costs.

Care Guidance services are designed to ensure that issues are proactively identified and each interaction moves a patient towards positive action. Care Guidance accomplishes this through:


1. Trained Care Guides – non-clinical persons hired, trained and managed by Guideway Care to provide effective, efficient peer-to-patient outreach.


2. Disease-Specific Protocols – a library of evidence-based protocols and structured workflows targeted to each condition to ensure that the factors influencing individual patient health and well-being are addressed and no one falls through the cracks.


3. Comprehensive Care Guidance Technology – a comprehensive technology platform that ensures Care Guides provide the right intervention at the right time, every time, and supports patients with reminders, prep instructions, education and more.
