CancerNAV is a clinical application used to support patient navigators as they manage the treatment path for their oncology patient population through the health system. CancerNAV was originally developed at the Cleveland Clinic and is proven to increase navigator efficiency and improve patient access and overall quality of care.
About Family Care Path:
Family Care Path was formed in 2016 as a spin-off company from Cleveland Clinic Innovations with a mission to bring patient-benefiting products from the academic institute to market. They founded the company to introduce the web-based clinical decision support applications. CancerNav based on oncology navigation practice guidelines developed by the Tausig Cancer Center at Cleveland Clinic CancerNav allows a clinical or lay navigator to assist a patient through the complex care continuum of cancer treatment. The web-based SaaS program is highly customizable and can be integrated with Epic and is an app on Cerner. CancerNav is available as an app on the Salesforce.com HealthCloud for ease of use in the EMR environment. .
Product Description:

Every 3 weeks, a cancer patient in chemotherapy spends about 8 minutes with their doctor. The data from the following 30,000 minutes is completely lost. We aim to quantify and utilize that data to provide better care.

Comprehensive Data Leads to Better Decisions

Precision PS is a clinical trial that uses an IoT approach to quantify a patient’s performance status by comparing lontitudinal data with physician assessed performance status. This is our first step in utilizing the other 30,000 minutes.

Objectively Measuring Performance

Cansera combines the Microsoft Kinect, wearable activity trackers, blue-tooth enabled scale, and electronic patient reported outcomes to give comprehensive information. We collect data through clinical trials of outpatient monitoring with IoT devices and 3D-motion-capture of performance tests.

REMOTE PATIENT MONITORING

A Continuous Monitoring Approach to Patient Care

We are using a whole range of interconnected sensors and devices to continually monitor the well-being of patients. With data collected from symptom monitoring and objective measures, we can create a safety net for cancer patients at home.

Enable Remote Clinical Trials

Exploratory analysis from our previous clinical trial suggests that lower average nonsedentary hours (measured using wrist-worn activity trackers) were associated with higher unplanned healthcare encounters (UHE). PS assessment in men was improved with the addition of patient reported outcomes (PRO). In women, body weight was the best predictor of unexpected medical events.
