Validic’s HealthBridge™ patient mobile application is available on the Apple App Store and Google Play store. 

The HealthBridge™ app serves as the connection between patients and their care team, allowing them to easily connect their personal health devices, review their device readings, and stay connected to their program provider. The HealthBridge™ app protects personal health information by only transmitting data to care teams from device connections that match the program in which a patient is enrolled.

The application takes a “set-it-and-forget-it” approach, with new readings automatically shared with healthcare providers - no input required, no need to open the mobile application, data transmitted securely in the background. Our health IoT platform and deterministic rules engine processes and analyzes the data in near-real-time and triggers relevant alerts and notifications for care teams. The digestible visualizations, alerts, and notifications give care teams near-constant visibility into a person’s health status for more timely interventions and personalized treatments or behavioral recommendations.
