Luma Health's Recall Management product expertly streamlines the process of patient recalls, ensuring timely follow-ups and continuity of care. This solution automates the identification and notification of patients due for appointments, screenings, or treatments, enhancing efficiency and patient engagement. It personalizes communication, sending tailored messages via patients' preferred channels, which significantly improves response rates and adherence to recommended healthcare schedules. Luma Health's Recall Management system is a vital tool for proactive patient care management, fostering a more connected and effective healthcare experience.

About Luma Health:

Luma was founded on the idea that healthcare should work better for all patients. Instead of a disconnected experience, where patients are forced to be their own healthcare advocates and provider teams struggle to reach their patients, every point along the care journey should be simple, seamless, and effective.

Luma’s Patient Success Platform™ empowers patients and providers to be successful by connecting and orchestrating all the steps in the patient journey, along with all the operational workflows and processes in the healthcare ecosystem.

Headquartered in San Francisco, Luma serves more than 650 health systems, integrated delivery networks, federally qualified health centers, specialty networks, and clinics across the United States, and today orchestrates the care journeys of more than 35 million patients.

Use Cases

Description:

33% reduced time to treatment

Sooner treatment means less patient anxiety and has been shown to improve outcomes. Stressed patients require additional resources, placing added burden on the healthcare system.

61% increase in patients per navigator

CancerNAV allowed navigators to work with significantly more patients while at the same time improving treatment. This had a positive effect on job satisfaction for the navigator.

26% decrease in patient leakage

Well-cared-for patients stay with the program, retaining revenue for the health care system, and likely leading to increased “word of mouth” referrals of new patients.

CancerNAV enhances the navigator’s ability to identify, track, manage interventions, and provide follow-up for the oncology patient population, in a seamless and real-time basis. Time to treatment is reduced, which has been demonstrated to reduce patient stress and improve outcomes. Less stressed patients utilize less services, and are more likely to stay with the program, improving the bottom line. With CancerNAV, the navigator can provide these improvements to patient care while handling more patients than before, improving productivity and providing a positive work experience for these important knowledge workers.
